Summary
Job Summary: The Manager of Case Management (or Case Manager) will coordinate patient care between different departments. They will assess patient needs, develop care plans, coordinate services with other care providers and monitor the outcomes of treatment. The responsibility of a healthcare Case Manager is to ensure that each patient has access to the best possible care by coordinating with physicians, hospitals, third-party providers, and the patient’s families. The Case Manager will identify gaps in care plans and work with other healthcare providers to address these gaps. Will work with patients and their families to decide on supportive treatment that are best for them. Works with doctors and other healthcare professionals to coordinate appointments. The Case Manager will fill out paperwork and communicate with insurance companies. While helping to determine a plan for a patient’s care by assessing their health needs. Helps in establishing a diagnosis through discussions with the patient and their family members. Provides referrals for treatment or support services.
Education/Experience: Must be a Licensed Registered Nurse in the State of Kansas. Certified in CPR. Bachelor’s degree in nursing with MS preferred. Associate's degree in nursing with experience. 5 years management experience or equivalent combination of education and experience.
